Your First Visit

Arrive early to complete your paperwork

If you are visiting Coastal Vein and Vascular Specialists for the first time or for a new condition or diagnosis, please arrive 15 minutes before your scheduled appointment time to complete all necessary paper work and allow time for registration. Our Patient Forms are available online for your convenience. Please print and fill out these forms before you arrive. If you don’t have a printer, no problem – we will provide these forms for you upon your arrival 15 minutes prior to your scheduled appointment time.

To assist the physician with your care, patients are asked to bring the following to their first office visit:

  • Drivers License or other form of Identification

  • All Medical Insurance Cards

  • List of current medications

  • Name, address, and phone number of all pharmacies you use – including all mail order pharmacies

  • Co-Payment, if applicable – we accept credit card or debit cards only. No CASH or CHECKS will be accepted

  • Any pre-registration forms you downloaded from our website and completed at home

You should expect to receive an ultrasound in addition to a consultation with our board-certified vascular specialist.

Please notify us of any changes to your name, address, telephone numbers, email, and insurance carriers as soon as they occur so we can keep your records current. We will require all information to be updated annually.

If you are unable to keep a scheduled appointment, or if you need to change your appointment, please notify us at your earliest convenience.
